To get a free updated Adobe Acrobat Reader, go the www.adobe.com and look for icon that says free Acrobat reader. Then follow the download and installation instructions.

These forms are in a fill-in PDF format. To use the fill-in form, tab or left mouse click to the required fields and fill  in on screen and then print out. The only requirement is that the forms be signed after printing if required (youth campers & SWAT). Failure to sign the forms properly will invalidate the application and result in the camper being denied access to the camp program (New York State Law). The fill in forms can be printed out and used as a form to be filled in with pen or pencil. Please try to use the fill-in function as the forms can be read much easier this way.

For all those under the legal age (18yrs old):
In order to attend a camp function, all six forms must be completed and mailed to the camp manager (with signatures where required!) CCA can not legally admit any camper under the legal age (18yrs old) who has not completed these requirements.
